We take your health and the health of our staff very seriously. We ask that you help us protect you, your family, and our staff by following the guidelines that we have been advised by the Traverse City Health Department and other experts to follow:
1) Everyone in the office must have a mask or face covering on: We ask that everyone wear a mask or face covering to their appointment. If you do not have one, we will do our best to provide one, however we cannot guarantee that we will have one available for you. If you do not have a mask on, we will need to reschedule your appointment.
2) If you have traveled out of state in the past 14 days, please reschedule your appointment: In an effort to minimize the risk of spread of COVID19 from "hot spots," please do not schedule your appointment until you have been in the Traverse area for at least 14 days after returning from out-of-state travel, both domestic and international.
3) Please reschedule if you are symptomatic: Please, please...if you have ANY symptoms, such as cough, fever, chills, headache, or feel sick in any way, please call to reschedule your appointment.
4) Patients only, please: We ask that only the patient enter the office, even for signing in, as we are trying to limit the number of people in the office at any given time. If you are brining a minor child to their exam, one parent or guardian will be allowed to accompany the child for the exam.
5) Please consider filling out paperwork beforehand: In order to increase exam efficiency, we ask that you consider printing and filling out our intake form and brining it with you to your appointment. We have provided a link to our privacy agreement
